Qualifications

Candidates must possess a pharmacy degree from an ACPE-accredited institution with completion of a PGY-2 ambulatory care residency program, PGY1/2 HSPAL program, or equivalent experience. Leadership experience required. Three years healthcare supervision preferred. Pharmacy licensure, or eligibility, in Indiana required.
