    Are you a compassionate Registered Nurse who thrives in a fast-paced, patient-centered environment? WakeMed Physician Practices is seeking an Ambulatory Care Nurse I (ACN) to join our growing Urology team in North Raleigh. This role offers the opportunity to build meaningful relationships with patients while providing expert clinical support and care coordination throughout their healthcare journey.

    As an Ambulatory Care Nurse, you will serve as a vital liaison between patients, providers, caregivers, and the interdisciplinary care team. You will play a key role in triaging patient needs, delivering education, coordinating care plans, and ensuring patients receive timely, high-quality care in the outpatient setting. This position is ideal for a nurse who enjoys combining clinical expertise with strong communication, critical thinking, and patient advocacy skills.

    What You'll Do

    • Coordinate and manage patient care across the continuum of treatment for urology patients.
    • Triage incoming patient concerns, assess clinical needs, and collaborate with providers to ensure timely interventions.
    • Educate patients and families regarding diagnoses, treatment plans, medications, procedures, and follow-up care.
    • Identify patients who may require additional care management services and facilitate appropriate referrals and resources.
    • Monitor patient progress, evaluate outcomes, and adjust plans of care in collaboration with providers and the healthcare team.
    • Advocate for patients and families while fostering positive, trusting relationships.
    • Document assessments, interventions, and patient communications accurately within the electronic medical record.
    • Support quality initiatives, patient experience goals, and operational excellence efforts across the practice.
    • Collaborate with physicians, advanced practice providers, medical assistants, and community resources to optimize patient outcomes.

    Preferred Experience

    While new graduate nurses are encouraged to apply, candidates with experience in any of the following areas will be highly valued:

    • Ambulatory or outpatient nursing
    • Urology, surgical, oncology, nephrology, or specialty practice settings
    • Telephone triage and patient care coordination
    • Chronic disease management
    • Electronic medical record documentation
    • Patient education and care planning
    • Care management or case management activities
