Position Summary

Responsible for coordinating and managing patient care through collaboration with medical providers and interdisciplinary teams to achieve optimal health outcomes. Provides case management for high-risk and complex patients, ensuring timely, efficient, and patient-centered care across the continuum.

This role includes care coordination, patient education, referral tracking, and participation in multidisciplinary care planning. The RN also performs clinical nursing duties, including assessment, documentation, medication administration, triage, and emergency response in a fast-paced clinical environment.

Promotes patient advocacy, safety, infection control, and continuous quality improvement while maintaining compliance with organizational standards and competencies.

Key Responsibilities

  • Collaborate with providers and care teams to develop individualized patient care plans
  • Coordinate multidisciplinary care conferences for high-risk or complex patients
  • Manage acute, chronic, and preventive care needs using EHR tools, reminders, and outreach
  • Participate in case management reviews with multidisciplinary teams
  • Advocate for patients and families to support informed decision-making and self-care
  • Coordinate referrals within SEARHC and external facilities, track progress, and resolve delays
  • Provide status updates on referrals and care outcomes to providers
  • Support discharge planning and care coordination across teams
  • Perform clinical nursing duties including assessment, documentation, education, and medication administration
  • Respond to triage needs and emergency situations as required
  • Maintain competencies, safety, and infection control compliance
  • Participate in team improvement initiatives and case management development
  • Maintain a safe, organized, and well-equipped clinical environment
  • Perform additional duties and special projects as assigned

Required Qualifications

Licensure & Certifications

  • Active, unrestricted Registered Nurse (RN) license in Alaska (required at time of application)
  • Current BLS certification required
  • National Case Management certification preferred

Education

  • Graduate of an accredited school of nursing required
  • Bachelor's degree in Nursing preferred
  • High school diploma or equivalent required

Experience

  • Minimum 1 year of case management experience within the last 2 years (required)

Compliance Requirements

  • Proof of current influenza vaccination required
