We've got a great opportunity for an Advanced Practice Psychiatric Nurse (PMHNP) to join us at our new integrated family care clinic, Health Solutions Family Medicine at Perry Hall, located at 1591 Taos Rd. in Pueblo, CO.
What You'll Be Doing
As a staff PMHNP with the Adult Outpatient, you would be providing strength-focused, evidence-based, and culturally sensitive treatment. Creating and sustaining positive, compassionate interactions with patients and providing high quality care will be central to the work you do. You would conduct assessments, manage cases, and provide supportive therapy with utmost kindness and care. Your attention to detail and problem-solving skills will be important to your responsibilities related to evaluating and managing medications. Furthermore, you'd have the distinct opportunity to work closely with a multidisciplinary team of Health Solutions professionals to serve our community.
Specifically, the Advanced Practice Psychiatric Nurse responsibilities under the administrative and clinical supervision of the Chief Medical Services Officer.
Administrative Duties
• Maintains accurate and current records with 100 percent compliance with Health Solutions' standards. • Uses electronic health records and electronic prescribing accurately and facilely to promote patient care.
Clinical Duties
• Performs psychiatric face-to-face assessments using formats established by Health Solutions. All assessments will include, at a minimum, a psychiatric history, medication history, mental status examination, psychiatric formulation, diagnosis in current DSM format, and a written plan of treatment. • Manages medication face-to-face follow-up visits for a full caseload of clients. • Provides as-needed emergency assessment, consultation, and medication prescribing based on availability of other providers. • Consults with other healthcare providers about relevant psychiatric/medical services and issues and assists in coordination of community-based services for attended clients. • Collaborates with professional colleagues, including physicians, nurses, and clinicians as required by Colorado Statutes related to professional collaborative relationships. • Provides active involvement in and completion of required quality assessment activities as required of all equivalent providers. • Attends scheduled medical staff clinical and administrative meetings and accepts assignment to applicable committees as required of all active medical staff. • As defined in applicable Colorado Statute, the scope of prescriptive authority for prescription drugs and controlled substances shall be limited to those patients within the incumbent's area of practice, training, and expertise. • Ability to lift 20 lbs. • Performs other duties as assigned by the Chief Medical Services Officer.
Physical Requirements
Requires standing, sitting, driving, and walking for prolonged periods of time. Occasionally lifts and carries items up to 20 pounds. Frequently exposed to stressful or emergent situations. Frequently operates a computer and other office equipment to record health data. Constantly communicates with clients and others. Occasionally drives between Health Solutions facilities or for other assigned duties. Requires occasional driving in adverse weather conditions.

What We're Looking For-The Must-Haves
Licensed Advanced Practice Nurse in Colorado in good standing. Graduate degree in nursing from an accredited academic institution Listed on the Advanced Practice Registry as Psychiatric/Mental Health Nurse Practitioner or Clinical Nurse Specialist. Prescriptive authority as defined by applicable state statute. Meets all State of Colorado statutory requirements including C.R.S. Title 12 Article 38, as amended and applicable, and Chapters XIV and XV of the Rules and Regulations of the Colorado Board of Nursing, and other applicable State and Federal requirements in effect during employment. Experience working successfully with a multi-disciplinary team, especially in an integrated primary care setting. Ability to work in a fast-paced primary integrated healthcare practice. Experience providing clinical services to patients of all ages. Experience and comfort working with a wide range of behavioral health conditions and presentations to include substance use disorders. Experience working with severe and persistently mentally ill adults, and experience with various behavioral health diagnoses including substance use. Good communication skills, both verbal and written. Exceptional time management skills. Flexibility and ability to easily adapt to change. Well-developed documentation skills. Computer proficiency. Valid Colorado driver's license, and access to reliable transportation during work hours. CPR/First Aid
What We'd Like to See in You-The Nice-to-Haves
Minimum two years supervised experience with use of drugs used within the scope of normal psychiatric practice. Advanced knowledge of principles of psychiatric nursing. Advanced knowledge of medication/medication and medication/food interactions. Advanced knowledge and experience in psychiatric assessment and ongoing psychopharmacological treatment. Spanish language skills
