What You’ll Do<\/span>
<\/p>

In this role the NP/PA will provide comprehensive healthcare services including assessment, diagnosis, treatment, preventive care, and chronic disease management to patients with a wide range of healthcare needs and conditions. The provider works collaboratively with physicians and the care team to deliver high -quality, patient -centered care.<\/span>

Key Responsibilities<\/span>
<\/p>

  • Conduct patient assessments and physical examinations<\/span>
    <\/li>
  • Diagnose and treat acute and chronic conditions<\/span>
    <\/li>
  • Order, interpret, and review diagnostic tests and imaging<\/span>
    <\/li>
  • Develop and implement treatment plans<\/span>
    <\/li>
  • Prescribe medications in accordance with state regulations<\/span>
    <\/li>
  • Provide preventive care, screenings, and health education<\/span>
    <\/li>
  • Manage chronic diseases and monitor patient outcomes<\/span>
    <\/li>
  • Coordinate referrals and follow -up care<\/span>
    <\/li>
  • Maintain accurate and timely medical documentation<\/span>
    <\/li><\/ul>

    What We Are Looking For<\/span>
    <\/p>

    Education, Experience, Certifications and Skills<\/span>
    <\/p>

    • Current NP or PA license in the state of Oregon<\/span>
      <\/li>
    • Graduate degree from an accredited NP or PA program<\/span>
      <\/li>
    • National certification as an NP or PA<\/span>
      <\/li>
    • DEA registration (or eligibility)<\/span>
      <\/li>
    • 3+ years of NP/PA experience, primary care experience preferred<\/span>
      <\/li>
    • Current BLS & ACLS certifications<\/span>
      <\/li>
    • Strong clinical, communication, and patient care skills<\/span>
      <\/li>
    • Experience with electronic health records (EHR)<\/span>
      <\/li>
    • Ability to work independently and collaboratively in a team environment<\/span>
