Position Description:

Penn Medicine Advanced Home Health (PMAHH) is searching for a full-time experienced Nurse Practitioner/PA to join their team. PMAHH is a line of service that provides care at home in the post-acute setting eg after early hospital discharges or after direct community referrals (PCPs or Penn Home Health). This team supports patients to stabilize at home with close monitoring, thus avoiding unnecessary hospitalizations and ED utilization. Infusion/IV treatments are offered under PMAHH with the support of the Penn Infusion Team, aiming at stabilization of decompensated chronic diseases. The team works close with the involved specialists (Cardiology, Pulmonary, ID).

The patients in this program receive care in their home. The provider's monitoring is mainly through telehealth visits, after an initial in person visit. The telehealth visits are supported by in person presence of Penn Home Health nursing, who are with the patient during the visit, reporting updates and exam findings. Patients benefit from this line of service by close monitoring, access to clarify treatment approach, empowerment of active participation in decisions (from treatment to goals of care), all within the comfort of their own home. 

The ideal APP candidate would possess excellent clinical assessment, decision making and communication skills and team player work ethic.

This role would require the APP to visit the patient’s home for the initial admission visit and in the event of decompensation. Mileage is tracked and reimbursed.

Role Review:

  • Consults with patients to determine the appropriate course of treatment. 
  • Recommends and orders tests to determine extent of illness or help diagnose condition. 
  • Monitors lab results, medication adherence and medication toxicities. 
  • Prescribe or administer treatment, therapy, medication, vaccinations, and other medical care to treat or prevent illness, disease, or injury. 
  • Explains and discusses procedures, test results, or prescribed treatment with the patient. 
  • Coordinates work with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers. 
  • Refers patients to medical specialists when necessary. 

Hours: Full-time position (40 hours/week) Monday-Friday. Call is shared with other providers and is assigned on a weekly basis. The expected call is one week per month. Call is light and is only from Penn Home Health nursing should any concerns arise during weekend visits or after hours calls to Home Health.

Position Requirements: 

  • Current Licensure as a Nurse Practitioner or PA.
  • Valid, unrestricted Federal Drug Enforcement Agency (DEA) number
  • Participation as a provider in good standing in the Medicare, Medicaid, and Tricare programs and such other third-party-payment programs, including managed care programs in white LGMG participates.
