Job Summary/Objective:

The Advanced Practice Provider collaborates with the medical staff to deliver comprehensive medication assessments and treatments to individuals accessing services through the outpatient medication management clinic and the Certified Community Behavioral Health Clinic (CCBHC). This role involves conducting face-to-face medication reviews with customers receiving ongoing mental health treatment through Summit Pointe’s service continuums.

Essential Duties and Functions:

  1. Conducts nursing evaluations and assessments according to service authorizations, within specified agency guidelines, and at designated locations.
  2. Evaluation, administration, and monitoring of medications as ordered by the physician.
  3. Assists the physician with medication reviews.
  4. May assist with calling in prescriptions to designated pharmacies written by Summit Pointe physicians.
  5. Assists physicians in the completion of initial psychiatric evaluations – with the physician conducting the psychiatric portion of the evaluation and prescribing medications accordingly.
  6. Assists the physicians by completing abnormal involuntary movement scales and other assessments, providing side effect education and completion of medication consents.
  7. Reviewing and signing off on health history screens. Issues of concern shall be brought to the physician’s attention.
  8. Assists the pharmacist in maintaining and monitoring medication samples contained in the medication room.
  9. Utilizes software used by the organization for monitoring of medication usage and trends as well as the electronic medical record.
  10. Participates in programs of quality assurance or utilization review including, but not limited to, ensuring compliance with National Patient Safety Standards as outlined by the organization’s accreditation resource.
  11. Maintains comprehensive medical records with all documentation completed within organization guidelines. Maintains record compliance within agency requirements and standards utilizing electronic/hard copy record formats.
  12. Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing medical education (per licensure), and professional improvement practices.
  13. Exercise sound, competent independent professional judgement regarding care and treatment of customers.
  14. Participates in corporate quality improvement and problem-solving activities.
  15. Maintains confidentiality and privacy within MDHHS, state and federal guidelines.
  16. Utilizes knowledge of CARF, CMS, MDHHS and Managed Care standards.
  17. Performs other duties as required.

Qualifications:

Educational

  • Licensure within the State of Michigan as a Licensed Registered Nurse with Certification as a Nurse Practitioner or Physician Assistant; and must obtain and maintain Controlled Substance Registration (DEA).
  • Must obtain and maintain CPR certification.

Basic Qualifications

  • Good organizational, verbal/written communication, computer, analytical and problem-solving skills.
  • Self-directed and resourceful.
  • Two years of experience within a medical setting, behavioral healthcare setting preferred.
  • Experience working with children preferred.
  • Specialized training, skills, and abilities: Lived experiences with behavioral health issues are desired.
  • One year of computer experience – Word, Excel, and electronic medical record preferred.

  • Professional office environment that generates noise from computer equipment/peripherals.
  • Possible exposure to health risks (violence and infectious diseases).
  • Sometimes lifts, carries, or moves objects up to 25 lbs.
  • May be required to work variable hours/shifts.
