Location: Green Cove Springs, FL (suburb of Jacksonville) 

Facility Name: Ascension St. Vincent's Primary Care - Lake Asbury

Schedule: Full-Time

Sign-on bonus and relocation assistance

  • Help establish a new clinic serving the community
  • Practice setting: outpatient
  • Access to [Dedicated MA | Support staff | Public Service Loan Forgiveness]
  • Employed position within a nonprofit health system
  • Nurse Practitioner or Physician Assistant
  • Deliver compassionate, evidence-based medical care, focusing on comprehensive patient evaluation and treatment.
  • Ensure seamless patient care coordination and compliance by providing thorough and timely medical documentation.

What You Will Need:

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Valid state license required specific to the state in which the associate works.

Education:

  • Master's degree of Nursing required.
