MyCare Medical is actively seeking a full-time, certified Nurse Practitioner to join our team in Jupiter, FL. We are a medical practice family specializing in value-based care, and delivering the highest quality care to our patients at the lowest costs available to them. About this Opportunity:
- Monday - Friday, 8am-5pm
- Rotating and flexible on-call schedule.
- Average daily volume is 12-15 patients per day
- We use eClinical Works (eCW), along with partnering programs to ensure ease of use for our patients and practices.
- We focus on continuity of care, health maintenance, and disease prevention, working alongside patients and their families to prevent, diagnose, treat and manage a Medicare Advantage population with multiple comorbidities.
- Integrated team-based care: Working as a multidisciplinary team to improve patient outcomes by meeting their needs and preferences.
- Prioritizing preventative care and proactive management of chronic conditions to improve patient outcomes, decrease patients costs, and increase revenue for the practice.
- Providers must be comfortable managing a primarily Medicare-aged patient population.
