Job SummaryThe APP Specialty at Prevea is an Advanced Practice Nurse NP or Physician Assistant PA who provide specialized healthcare services tailored to the specific department. This role operates within the scope of the specific specialty department and includes but isnt limited to collaborating and coordinating with physicians and other health professionals regarding clinical practice education consultation research and program development. What you will doProvides assessment evaluation and management to patients that may include but is not limited to minor acute chronic & episodic illness & injuries. Including performance of follow up exams if required.Collaborates with patients to review their health background and wellness provide health counseling and education to determine physiological emotional and environmental needs using this information to advise patients and their families on future health plans and illness prevention.Provides follow-up services as appropriate to assigned specialty which may include the ordering of appropriate diagnostic and laboratory tests the prescription of pharmacological agents and other non-pharmacological treatment modalities.Maintains and reviews patient records charts and other pertinent information documenting all details in the electronic medical record.Collaborate with physicians and other providers to develop and implement patient care plans to ensure the best possible care and outcomes of patients.Provides case management services to assist clients and families with the arrangement for or refer patients to needed services.Education QualificationsGraduate of a masters level nurse practitioner program OR from and accredited physician assistant program. RequiredExperience Qualifications1-3 years work experience in a clinical setting within a health care organization PreferredSkills and AbilitiesUnderstanding of the specialty departments information and techniques necessary for diagnosing and treating human injuries and diseases including symptom recognition treatment methods medication properties and interactions as well as preventive healthcare practices.Excellent interpersonal skills and behaviorsAbility to form workpatient relationships with patients and peers.Ability to prioritize work and meet changing demands.Ability to handle and process confidential information.Fosters teamwork through effective written and verbal communication.Licenses and CertificationsIndividual must posses either the licensescertification details of a Nurse Practitioner or a Physician Assistant. ALL are required to have CPRBasic Life Support.Nurse Practitioner Registered nurse in the state of Wisconsin with a certification through American Nurses Credentialing Center ANCC or American Academy of Nurse Practitioner Certification Board AANPCB. Also certified in the State of Wisconsin as an Advanced Practice Nurse Prescriber or certification eligible obtaining certification at the first opportunity Upon Hire RequiredPhysician Assistant Licensed by the state of Wisconsin as a Physician Assistant.
