As a care leader, you'll manage a diverse patient panel, collaborate with a multidisciplinary team, and support health outcomes through outpatient care, disease prevention, and health maintenance.
Responsibilities include maintaining medical records, referring to specialists, participating in team huddles, and making complex clinical decisions independently.
Requirements include 2+ years of experience (preferably in geriatrics), active state licensure, board certification in Family, Adult, or Geriatrics, DEA license, and bilingual skills (English/Spanish) preferred. Strong leadership, communication, and proficiency in Microsoft Office are essential.
