This position will provide services for the Lac Vieux Desert Health Center.

Duties: • Function independently as a certified Nurse Practitioner for a patient population in collaboration with the staff physician at the Lac Vieux Desert Health Center • Provides primary health care services emphasizing the promotion of health and the prevention of disease • Independently assesses acute and non-acute clinical problems and illness • Performs and documents physical assessments and patient histories, analyzes trends in patient conditions, and develops, document and implements a patient management plan in response to the data obtained • Works closely with specialists in determining treatment plans for patients with needs outside of the scope of family practice • Manages actual and potential health problems, which include common diseases and human response to diseases with emphasis on prevention • Plans patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying physiological and/or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality, and support systems • Serves as patient advocate • Patient management includes the following: 1) Diagnosing and treating illness and injuries, acute and chronic disease conditions 2) Ordering and interpreting appropriate laboratory and diagnostic studies 3) Ordering of appropriate medication and treatments 4) Referring patients for consultation when indicated i.e. dermatology, neurology, cardiology, nephrology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, and dietary 5) In office procedures such as suturing, skin punch biopsies, birth control device implants, joint injections 6) Counseling and educating patients on treatment and prevention of health problems 7) Documentation through in-depth progress notes and dictation summaries • Participates in patient care conferences • Communicates patient management strategies to members of the patient care team • Collaborates with members of the multidisciplinary team to ensure that patient management strategies are successful in meeting patient care and research needs

Qualifications: • Graduate of an accredited School of Nursing • Graduate of an accredited Nurse Practitioner Program • Current Advanced Practice Registered Nurse (APRN) Licensure • Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) • Strong computer skills, including Word, Excel, and Power Point • Minimum of 3 years of Family-Practice or Internal Medicine experience in the Nurse Practitioner role • Must have strong and polished interpersonal, written, and oral communication skills.
