About the Role
The Advanced Practice Provider serves as a trusted clinician and clinical resource, delivering high-quality care through comprehensive patient evaluation, treatment, education, and follow-up. Working closely with physicians and multidisciplinary care teams, the APP ensures continuity of care from hospital encounters through outpatient communication and coordination.
This role blends clinical autonomy with strong team support and offers meaningful involvement in quality improvement, patient experience, and care coordination initiatives.
