Position Summary:
This unique Advanced Practice Provider (APP) position offers the opportunity to provide care across two complementary practice settings: the STAR (Support Team for Aftercare and Resources) Clinic and our Primary Care team.
The STAR Clinic is a post-discharge transitional care program focused on helping patients with complex medical and medication needs successfully transition from the hospital to the outpatient setting.
The APP will provide comprehensive follow-up care, medication management, care coordination, and facilitate connections to primary care and community resources to improve outcomes and reduce avoidable readmissions. In addition to supporting the STAR Clinic, the APP will practice within our Primary Care clinics, providing direct patient care to improve access, support same-day and follow-up appointments, and assist with provider coverage.
This structure allows the APP to develop and maintain an independent primary care patient panel while contributing to a collaborative, team-based care model. This dual-role position offers a diverse and rewarding practice with opportunities to build long-term patient relationships, improve care transitions, enhance patient access, and collaborate closely with physicians, advanced practice providers, nursing, pharmacy, and care management teams to deliver high-quality, coordinated care.
