Position Summary: LEAP is seeking an experienced, compassionate, and organized Adult Community Case Manager to join our Case Management team in Farmington, Maine. In this role, you will coordinate essential services and support for adults with intellectual- and developmental disabilities and/or autism. You will serve as a trusted advocate, helping individuals access resources, navigate MaineCare and waiver systems, and develop and implement person-centered plans that reflect their goals and dreams. This position involves a dynamic mix of community-based work, family engagement, and cross-disciplinary coordination.
