What You’ll Do
As an Admissions Registered Nurse (RN), you will:
- Conduct comprehensive Start of Care (SOC) assessments and complete accurate OASIS documentation in compliance with Medicare and agency standards
- Evaluate patients' clinical needs, functional status, and home environment to develop individualized plans of care
- Coordinate admissions and collaborate with physicians, therapists, case managers, and other interdisciplinary team members to ensure seamless patient care
- Educate patients and caregivers on diagnoses, medications, treatment plans, disease management, and home health services
- Monitor patient conditions during the admission process and communicate significant findings or changes to physicians and clinical leadership
- Complete all admission documentation accurately and timely within the electronic medical record (EMR) while maintaining regulatory compliance
What You’ll Need
- Graduate of an accredited Registered Nurse (RN) program
- Active South Carolina RN license or Compact License
- Minimum one year of nursing experience
- Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
- Familiarity with OASIS documentation preferred
- Previous home health experience highly preferred
