What You’ll Do 

As an Admissions Registered Nurse (RN), you will: 

• Conduct comprehensive Start of Care (SOC) assessments and complete accurate OASIS documentation in compliance with Medicare and agency standards
• Evaluate patients' clinical needs, functional status, and home environment to develop individualized plans of care
• Coordinate admissions and collaborate with physicians, therapists, case managers, and other interdisciplinary team members to ensure seamless patient care
• Educate patients and caregivers on diagnoses, medications, treatment plans, disease management, and home health services
• Monitor patient conditions during the admission process and communicate significant findings or changes to physicians and clinical leadership
• Complete all admission documentation accurately and timely within the electronic medical record (EMR) while maintaining regulatory compliance

What You’ll Need 

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred 
• Familiarity with OASIS documentation preferred 
• Previous home health experience highly preferred
