We are seeking an experienced Admissions Nurse (RN) who is passionate about improving lives, building meaningful community partnerships, and ensuring residents receive the right care at the right time.
$94,000-$105,000 per year based on experience!
As the Admissions Nurse, you will serve as both a clinical leader and community ambassador. You will be instrumental in helping residents and families navigate challenging healthcare transitions while collaborating with hospitals, physicians, case managers, and community providers to ensure a smooth and successful admission process.

What you'll do:
- Review, evaluate, and process admission referrals in a timely manner.
- Conduct comprehensive nursing assessments for prospective and high-risk admissions, including medication reconciliation.
- Ensure appropriate clinical placement and coordinate bed assignments based on resident needs and acuity.
- Collaborate with physicians, case managers, discharge planners, social workers, and healthcare providers to facilitate smooth transitions into the facility.
- Assist with admission orders, nursing assessments, care plans, and other clinical requirements.
- Verify and confirm all physician orders prior to admission.

Who We’re Looking For:
- Current Registered Nurse (RN) license in the State of Hawaii and in good standing.
- Minimum two (2) years of Long-Term Care (LTC) nursing experience.
- Experience in Community-Based Care, Care Coordination, Transitional Care, Case Management, or related healthcare settings.
- Strong understanding of skilled nursing operations and post-acute care services.
- Excellent clinical assessment and critical thinking skills.
- Exceptional communication, relationship-building, and customer service abilities.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Strong organizational and time-management skills.
- Proficiency with electronic medical records and Microsoft Office applications
