Director of Admissions/Case Management

Reports To: Administrator

Supervises: Admissions Coordinator/Marketers

Coordinates With: Department Heads

Education/Experience/Qualifications:

  • Bachelor's Degree
  • Good oral and written communication skills.
  • Organizational ability.
  • Interview and assessment skills.
  • Knowledge of entitlements and community resources.
  • Knowledge of health care and gerontology issues.

Physical/Mental Qualifications:

  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately to ensure performance of job duties.
  • Mentally alert and emotionally stable.

Scope: Maintain day-to-day functions of the Admissions Department.

Duties and Responsibilities:

  • Review all applications and PRIs for Admission and forward to review as appropriate.
  • Conduct tours and provide information about Avalon Gardens to applicants, hospital discharge planners and community agencies.
  • Interview applicants and family members as needed.
  • Process and follow up on Medicaid applications, VA applications, Social Security applications, recertifications and other entitlement applications.
  • Complete daily census and distribute to all departments.
  • Maintain Bedhold, TLOA documents and correspondence.
  • Coordinate Comprehensive Care Plan (CCP) meeting times and distribute lists.
  • Complete CCP letters for distribution to sponsors and residents.
  • Admit residents in accordance with established policies and procedures.
  • Upon admission, complete, review and explain admission documents to residents and/or sponsor obtaining signatures as necessary.
  • Complete MDS+ sections as assigned.
  • Complete annual screen evaluations on all residents.
  • Maintain waiting list and related correspondence.
  • Maintain Admission and Discharge Registers, as well as the PRI Log Book.
  • Other related duties as assigned by Supervisor.
