Description

Centralized Admissions Coordinator

Location: Office-based, Frankenmuth, MI
Full-Time | Salaried Position
Applications Accepted Through July 29, 2026

Be the Connection Between Families and the Care They Need

Every day, families across Michigan are faced with important decisions about care for a loved one. They need guidance, answers, and confidence that they're making the right choice. Hospitals need trusted partners who can respond quickly. Communities need someone who understands both the clinical and operational side of admissions.

That's where you come in.

As a Centralized Admissions Coordinator, you'll serve as a vital link between hospitals, referral partners, families, and Wellspring's skilled nursing communities. Working from our Frankenmuth office, you'll help evaluate referrals, coordinate admissions, and guide prospective residents through the transition into care.

This position supports all four of Wellspring's skilled nursing facilities throughout Michigan. While you'll have ownership of your work, you won't be doing it alone. You'll be part of a centralized admissions team alongside other dedicated Admissions Coordinators who work together to manage referrals, solve challenges, and support one another every day.

If you've built a career in skilled nursing, healthcare admissions, case management, insurance, or long-term care and still find fulfillment in helping people navigate complex healthcare decisions, this role offers the opportunity to make a meaningful impact while working in a collaborative and growing environment.

What You'll Do

Admissions Coordination & Referral Management

As a Centralized Admissions Coordinator, you will guide referrals from initial inquiry through successful admission, helping ensure residents receive the right care at the right time. You'll review and respond to referrals promptly while evaluating each opportunity from clinical, financial, and operational perspectives. Working closely with hospitals, physicians, discharge planners, families, and other referral sources, you'll coordinate the admissions process from inquiry through placement and help create a smooth transition into care.

Insurance Verification & Documentation

A key part of this role involves managing the administrative and financial components of admissions. You'll complete insurance verifications, pre-certifications, and required documentation while ensuring information is accurate, current, and compliant. Maintaining real-time referral updates and organized records will be essential to supporting efficient decision-making and admissions workflows.

Collaboration & Occupancy Support

You'll partner closely with Administrators and campus leadership across Wellspring's skilled nursing communities to support occupancy goals, census management, and payer mix objectives. As part of the centralized admissions team, you'll work collaboratively to solve challenges, share information, and ensure referrals are directed to the most appropriate community based on resident needs and organizational priorities.

Relationship Building & Process Improvement

Success in this role requires strong communication and relationship-building skills. You'll serve as a trusted resource for referral partners, residents, and families by providing timely follow-up, responsive communication, and exceptional customer service. You'll also monitor referral trends, evaluate conversion outcomes, identify barriers to admission, and contribute ideas that improve processes and overall admissions performance.

Who Thrives in This Role

Relationship-Focused Professionals

The strongest candidates enjoy building meaningful relationships with healthcare providers, hospitals, discharge planners, and families. They understand that admissions is about more than placement decisions; it's about helping people navigate significant life transitions with confidence and support.

Strategic Decision Makers

Successful Admissions Coordinators balance clinical knowledge with financial and operational awareness. They can evaluate complex information, assess opportunities objectively, and make informed recommendations that support both resident needs and organizational goals.

Organized Problem Solvers

This role is ideal for professionals who remain focused in a fast-paced environment where priorities can shift throughout the day. Thriving candidates are naturally organized, proactive, and skilled at identifying solutions to challenges while maintaining attention to detail.

Collaborative Team Members

Because this position supports multiple skilled nursing communities and works as part of a centralized admissions team, strong collaboration is essential. The best candidates take ownership of their responsibilities while actively contributing to team success and continuous improvement efforts.

  • Licensed Practical Nurse (LPN) in Michigan preferred.
