Salary Range: $60,000.00 To $75,000.00 Annually
Position Summary:
Responsible for clinical and financial evaluation of hospital and community referrals for admission and re-admission to St. John's Home, including all aspects of the process: screening, insurance verification and authorization, continuous communication with families & various community care partners, bed management, and oversight of the admission paperwork. Works in collaboration with other departments to provide support to residents and families, as well as maintain census and payer mix.
Qualifications & Education Required:
Bachelor's Degree in Social Work or Human Services OR Bachelor's Degree in Nursing or NYS RN Licensure OR Licensed Practical Nurse (LPN), with a minimum of three years' experience in LTC with experience with the admissions process
Preference for at least 2 years of experience in a skilled nursing facility with ability to prioritize and manage multiple tasks, often with interruptions, as well as familiarity with health insurances and Medicaid

Position Responsibilities:
Conducts thorough and timely review of hospital and community referrals, with attention to detail for a successful admission
Sets up appointments and provides tours for perspective applicants/families
Offers guidance regarding alternative choices or services in lower levels of care, if appropriate
Works with health insurances, Medicare, Medicaid and families to facilitate coverage and payment
Consistently communicates with the interdisciplinary team regarding new and re-admissions
Ensures the Business Office has accurate information about insurance and finances for applicants, monitors and updates bed hold status and social leave days for appropriate payment
Provides a welcoming and supportive environment to the prospective elder and their loved ones as they navigate the admission process
Assists with admission paperwork, as needed
Fosters cooperative relationships with external entities to maintain great working relationships
Data entry for all referrals, admissions and denials
This position is full-time, in-person
