Admissions Clinical Liaison

Lacon Rehab & Nursing is seeking an experienced Admissions Clinical Liaison for a full-time position.

The Admissions Clinical Liaison plays a key role in the admissions process by completing clinical intake evaluations, reviewing patient histories, coordinating insurance and authorization needs, and clinically approving admissions for post-acute care services.

Responsibilities:

• Complete admission and intake assessments

• Review medical records and patient histories

• Verify insurance coverage and manage prior authorizations

• Coordinate admissions with hospitals, families, and care teams

• Ensure required admission documentation is completed

• Support census growth and admissions workflow

Qualifications:

• Experience with insurance verification and prior authorizations preferred

• Knowledge of post-acute care, skilled nursing, sub-acute rehab, or rehabilitation settings

• Strong communication and organizational skills

• CNA background with admissions or insurance experience encouraged to apply

Compensation & Benefits:

• Salary up to $45,000 annually
