Transitions is a short-term residential program supporting individuals of all genders with alcohol and/or substance use disorders, serving as a transitional step between detox and long-term care. Funded by the Bureau of Substance Addiction Services and located in Boston, it follows a 'No Wrong Door Policy,' accepting clients from various settings such as hospitals, shelters, and outpatient programs.
- Assists in coordinating referrals, screenings, admissions, discharges, and tracking client data.
- Manages MIS data, maintains client records, and updates logs.
- Conducts outreach to referral sources and builds relationships with external agencies.
- Facilitates client orientation and monitors behaviors, intervening when needed.
- Completes billing, fiscal documentation, and participates in team meetings.
- Supports anti-racism efforts by promoting health equity and social justice.
- Requires a BA/BS with experience, or relevant education and experience, with strong communication skills, computer literacy, valid MA driver’s license, and bilingual abilities preferred.
