We are currently hiring an Admission Nurse (RN) to join our Transitional Care Unit (TCU). In this role, you'll be the first point of contact for newly admitted patients, ensuring a smooth transition into our community through comprehensive assessments, care coordination, and compassionate support.
Wage: $36.00-$43.93/hr.
Schedule: 11:30am-7:00pm
What You'll Do
Welcome and admit new patients, creating a positive and reassuring first impression
Conduct comprehensive admission assessments and nursing interviews
Review, clarify, and enter physician and nursing orders accurately
Initiate individualized care plans in collaboration with the interdisciplinary team
Communicate patient status, special care needs, and admission updates with nursing staff
Educate patients and families on the admission process, care expectations, and available services
Ensure admission documentation is complete and accurate within the electronic medical record (EMR)
Coordinate with physicians, therapists, and other healthcare professionals to establish treatment plans
Identify and communicate changes in patient condition or potential risks promptly
Collaborate with discharge planning and care teams to support a seamless patient experience
Serve as a liaison between patients, families, and the healthcare team throughout the admission process
Deliver exceptional customer service while supporting patients during their transition into care
What We're Looking For
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing
Experience using PointClickCare (PCC) or other electronic medical record (EMR) systems
Strong clinical assessment, communication, and customer service skills
Ability to prioritize, multitask, and work effectively in a fast-paced environment
CPR certification
Preferred:
Previous experience in transitional care, skilled nursing, or long-term care
Admissions, assessment, or case management experience
Knowledge of Medicare, Medicaid, and insurance requirements related to transitional care
Experience collaborating with interdisciplinary healthcare teams and discharge planning
Working Conditions
Ability to stand, walk, bend, reach, and move throughout the facility during the shift
Ability to lift, push, and pull up to 50 pounds
Ability to assist with patient transfers and mobility support as needed
Transitional Care Unit and skilled nursing environment with frequent patient interaction
Fast-paced, team-oriented setting requiring adaptability, organization, and strong critical thinking skills
Adherence to infection control, safety, HIPAA, and confidentiality standards
Variable schedule that may include weekends and holidays based on department needs
