We’re looking for a compassionate and organized Admissions and Discharge Specialist to help welcome new residents, support smooth transitions, and ensure every individual and family feels cared for every step of the way.
This role is key to creating a positive first impression and a lasting sense of trust. You’ll guide residents and their loved ones through the admission and discharge process, providing reassurance, communication, and a personal touch that reflects our facility’s values of kindness, respect, and quality care.
Work schedule is Monday through Friday 8:00am-4:30pm
Wage range based on experience: $23-26/hr.
What You’ll Do
- Assist residents/families/legal representatives with arrangements to ensure an efficient and orderly admission or discharge including paperwork, transportation, personal belongings, financial planning, needed referrals and emotional support and guidance
- Serve as advocate and facilitator for residents and families regarding questions, concerns, or problems that may arise within the resident's living situation at Evergreen
- Provide information and assistance with financial and medical insurance concerns and make appropriate referrals to community agencies
- Provide guidance to unit staff on assigned shift and follows through during issues and concerns while keeping the Unit Manager informed
- Coordinate and facilitate weekly resident care planning meetings. Provide current information on residents being reviewed at care planning
What We’re Looking For
- High school diploma or equivalent required; Associate’s or Bachelor’s degree in Human Services field from an accredited college
- 1+ year of experience in a long-term care setting preferred
- Excellent professional communication (oral and written), empathy, and problem-solving skills
- A team player who thrives in a close-knit, caring environment
- Proficiency in Microsoft Office
