Pikes Peak Hospice & Palliative Care has an immediate opening for an Admissions Coordinator II position.
LOCATION: Colorado Springs, CO
STATUS: Full-time
SCHEDULE: Monday-Friday, 9:00a - 6:00p, rotating weekends
PAY RANGE: $23.63 - $27.77 per hour
SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Overtime
MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile – one of the highest in the industry!

REWARDING WORK YOU WILL DO:
The Admissions Coordinator II performs professional work planning and coordinating supportive services to hospice and/or palliative patients and their families. Acts as a patient and family advocate with outside agencies to assure maximum care is received. Provides excellent service to internal and external customers in regards to admissions for Hospice and Palliative health services. Answers incoming calls and provides information regarding hospice, palliative and/or home health care programs and admissions criteria to external and internal customers.
- Provides information to patients, families, and facilities to explain care options including in the home, long-term care, assisted living care and/or hospice inpatient facility settings. Effectively works with discharge planners and referral sources.
- Collaborates with the Medical Community via phone, EMR system, email, etc. to ensure timely customer service and communication.
- Meets with patients and families to assess appropriateness for hospice or palliative care in the home, long-term care or assisted living facilities, or hospital setting. Effectively works with discharge planners and referral sources. Promotes holistic approach to assessment of patient care needs. Completes full Explanation of Benefits (EOB) as primary function of the position.
- Effectively provides information, communication and educates patients, families, physicians, and other referral sources regarding hospice and palliative services. Demonstrates appropriate referrals to community sources for financial, instrumental, and legal assistance.
- Works closely with Business Development and Clinical team/staff to ensure timely patient care.
- Efficiently and accurately completes all data entry. Initiates and presents admission paperwork to appropriate patients and families based on the need, payer source, financial benefit, and legal assistance.
- Collaborates with IDG members to ensure optimal patient/family care. Educates patients, families, staff, and community about End of Life issues.
- Verifies insurance coverage, obtains authorization for admission, orders durable medical equipment (DME), provides necessary documents, and forwards to appropriate hospice staff as back up to Insurance Specialist or as needed.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
- Minimum Education: Two (2) years college required. Degree from an accredited university in medical, business or human relations preferred.
- Minimum Experience: One/two (1-2) years of experience in a hospice or other clinical setting.
PHYSICAL REQUIREMENTS:
- Ability to lift/carry a minimum of 30 lbs.
