Summary:
- The Acute Rehab Clinical Liaison is responsible for generating admissions to Acute Rehab, through direct contact with external referral sources, including Princeton Healthcare System. Screens referrals for level of care, and collaborates with Physicians, financial counseling, and social work to coordinate appropriate care levels. The position can be held by an RN, PT, OT, SLP licensed professional.
Responsibilities:
Screens outside and inside referrals from physicians, case managers, social workers, family members, etc. for potential admission to the Acute Rehabilitation Unit.
Evaluates referrals based on the CMS 13 (Medicare) admission criteria, including clinical condition, mental and psychosocial needs, level of care, equipment needs, potential for discharge, and financial status taking into consideration the 60/40 CMS rule.
Evaluates referrals based on chart reviews, discussions with the acute rehab management team and physician, and meeting with patient/patient family.
Completes all pre-admission assessments. Maintains files.
Enhances relationships with existing referral sources and develops new relationships with physician groups and the outside facilities. Presents information about Acute Rehab to interested community groups, managed care organizations, hospitals, assisted living communities, etc…
Admissionso Coordinates daily with recommendations/decisions on admissions at daily census meeting. Assures complete admission assessments are available for census meeting.o Communicates the results of admissions decisions to referral source. As appropriate continues communication with patient on admissions decisions.o Works effectively and efficiently with Case Management team in expediting patient’s referral to community or facility based services in alignment with goals of cost effective and appropriate care/services.o Completes all work timely and establishes minimum 24-hour return call policy to referral sources.o Supports customer service, and performance improvement goals. Provides referral/admission statistics to clinical staff as needed.o Communicates pre-admission clinical needs to nursing, including setup days for dialysis/radiation therapy, O2, vacs, TPN, etc…o Meets departmental admission objectives in both census, and census mix.o When necessary, goes to outside referring facilities to determine appropriateness of referral and clinically assess patient’s
Marketingo Assists in development of marketing plan and identifying potential referral sources.o Meets established referral/admission/census targets.o Participates in Marketing lunches, dinners, etc…o Participates in in-house marketing meetings.o Actively participates in all CARF related activities.
Credentials:
- Basic Cardiac Life Support (Required)
- Driver's License (Required)
- RN or PT or OT or SLP (Required)
- Licensed Registered Nurse in the State of NJ or licensed PT, OT, SLP required.
Education or Equivalent Experience:
- Associate of Arts or Science (Required)

Equivalent Experience:
•1+ years Admission experience in Acute Rehabilitation
•Specific experience working with Medicare skilled care criteria, Medicaid, managed care, and private pay.

Salary Range:
$78,265.82 - $124,813.73/Annually based on 40 hours per week
