Job Summary and Qualifications

  • The individual is responsible for facilitating admissions of Intake patients from HealthONE emergency departments to behavioral health hospital services via management of phone triage. 
  • He/she will work closely with all healthcare providers to facilitate and coordinate efficient, effective, quality healthcare. 
  • The individual must demonstrate knowledge of the principles of growth and development across the life span; possess the ability to assess data reflective of the patient’s clinical status and interpret the appropriate information needed to identify each patient’s requirements related to his or her age-specific needs. 
  • He/she will utilize various computer technologies to ensure communication between Intake and emergency departments regarding the flow of mental health patients. 
  • He/she will implement and comply with all the policies, procedures and standards of the unit, department and the institution. 
  • The Intake Placement specialist must be available for 12-hour shifts with rotating weekends. 
  • The Placement specialist is responsible for providing services to patients who have complex psychosocial and discharge planning needs. 
  • In addition, the Placement specialist community assistance, and under the guidance of the HCAT Clinician, coordinates and facilitates the development of a Discharge Plan of Care for at-risk patient population with discharge needs. 
  • The Placement specialist also functions as a member of the Interdisciplinary Patient Care Team, who communicates and receives referrals from other patient care providers to help meet the needs of the patient and family for a safe discharge. 

 Special Qualifications:  

  • Excellent written and verbal communication skills, along with excellent interpersonal communication skills 
  • Knowledgeable of State of Colorado, Mental Health Services 27-65 statute and regulations as well as Alcohol and Drug Abuse Division 27-81-101 and 27-82-107 statutes and standards, and EMTALA 
  • Knowledgeable of behavioral health emergency populations, evaluation, consultation, and process 
  • Ability to perform multiple tasks simultaneously, apply critical thinking process 
  • Ability to work in high acuity and stressful environment 
  • Ability to engage multiple systems in disposition planning and implementation 
  • Respond to all customers using AIDET, iCARE, and knowledge with Trauma Informed Care  

What qualifications you will need:

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • High School Graduate / GED
  • Bachelor's Degree in Social Work or related field - strongly preferred
  • 1 year of mental health or crisis work - strongly preferred
  • Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
