ACTT Team Leader (LCSW / LCMHC / LMFT)

Pathways to Life, Inc. — Greenville

Schedule: Mon–Fri, 8:00–5:00
On-call: 24/7 rotation shared across 6-8

What this job looks like (week to week)

  • Caseload: 8–12 adults with serious mental illness (SMI) + substance use
  • Field work in homes, shelters, hospitals
  • Office/admin time for supervisions, documentation, etc.
  • Team: psychiatry, nursing, therapists, peer support
  • Crisis calls: part of rotating 24/7 coverage

What you do

Clinical

  • Complete biopsychosocial + substance use assessments
  • Build treatment plans with the ACTT team
  • Deliver therapy using:
    • Motivational Interviewing
    • Cognitive Behavioral Therapy
    • Relapse prevention

Field-based care

  • See clients in homes, shelters, ERs, and community locations
  • Run 2–4 person-centered meetings/week
  • Support stabilization (med adherence, coping skills, anger regulation)

Crisis response

  • Respond to behavioral health crises in the community
  • De-escalate situations

Team leadership

  • Guide 6–8 team members on clinical decisions
  • Coordinate with psychiatry, nurses, employment specialists, peer support
  • Review documentation to meet Medicaid + ACTT standards

Requirements

License (North Carolina)

  • LCSW, LCMHC, or LMFT

Experience

  • 1 year working with adults (paid or volunteer)
  • Direct work with SMI, substance use, or crisis populations

Other

  • Driver’s license + daily travel
  • Lift up to 25 lbs
