About ACTT
Assertive Community Treatment Team (ACTT) is an evidence-based, intensive community treatment model designed for adults with severe and persistent mental illness who may struggle with traditional outpatient services and are at increased risk for hospitalization, homelessness, crisis episodes, substance use, or incarceration.
Often described as a “hospital without walls,” ACTT delivers comprehensive behavioral health services directly in the community, including homes, shelters, hospitals, workplaces, and other community settings.
The goal of ACTT is to help individuals:
Remain safely in the community
Reduce psychiatric hospitalizations
Improve independence and quality of life
Strengthen daily living and coping skills
Achieve recovery and personal goals
Services are individualized, flexible, and centered on each member’s needs and strengths.
The ACTT Qualified Professional provides intensive, person-centered behavioral health interventions, crisis support, rehabilitation services, and care coordination in community and field-based settings.
The QP works closely with psychiatrists, nurses, therapists, peer support staff, and substance use professionals to support members with symptom management, independent living skills, medication adherence, housing stability, employment goals, and community integration.
