Assertive Community Treatment (ACT) Team Case Manager:
OnPoint is seeking an energetic, organized, committed professional to fill the position of full-time Assertive Community Treatment (ACT) Team Case Manager. As an ACT Team Case Manager with OnPoint, you will be responsible for providing support and opportunities for individuals with mental health, substance use, or co-occurring disorders in a community-based setting to include client homes and community locations. Provide case consultation and case management for the needs of ACT Team clientele and participate in team-model of care.
PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $57,948.80 up to $79,955.20 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
Bachelor’s degree in social work, psychology, or related field required.
One year’s experience working with adults diagnosed with an intellectual /developmental disability or mental health disorder required.
Must have a valid State of Michigan driver’s license, a good driving record, and valid proof of insurance required.
PREFERRED QUALIFICATIONS:
Community-based service delivery experience.
Working knowledge of issues and dynamics in the delivery of services to individuals with mental health, substance use, or co-occurring disorders.
Lived experiences with mental health, substance use, or co-occurring disorders valued.
RESPONSIBILITIES:
Individual and case management support of clients on ACT Team with mental health and co-occurring disorders.
Case management and case consultation across ACT Team, OnPoint departments, and community/natural supports for clientele.
Facilitate the person-centered planning process.
Provide Case Management Services to individuals on assigned case load through:
Transportation as required by treatment plan.
Development of Natural Supports including family, friends, religious organizations/leaders, neighbors, teachers, employers, and all others involved and interested in supporting the client. Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers.
Internal and external case collaboration and coordination, including gathering and exchanging relevant information.
Consultant to individuals and their support systems.
Documentation of services and supports in accordance with funding, regulatory, accreditation and best practice standards.
Monitoring support providers in the provision of services and implementation of person-centered plans.
Participation in clinical case conferences, staff development (in-services, conferences, professional organizations) and agency committees as assigned.
To provide support for after-hours/on-call as needed and scheduled for ACT. This is a shared responsibility across the ACT Team.
Performs other duties as assigned.
EQUIPMENT/TECHNOLOGY KNOWLEDGE:
Efficient in Microsoft Word and Excel.
Working knowledge of Electronic Medical Records (EMR) system.
Exceptional written and verbal communication skills.
