As the VISTA Performance Measurement Coordinator for our Health Navigation Services project you will: 1) Develop project data collection protocols and instruments, 2) Train staff on project data collection instruments and procedures, and databases management, 3) Collect project data and conduct performance measure on key project outcome indicators to demonstrate the project effectiveness and identify needed improvement measures, 4) Review the project staff monthly reports for completeness and accuracy, and provide feedback to them on any discrepancies identified and how to address them, 5) Elicit client feedback through client satisfaction surveys, 6) Analyze client's access and utilization of services information and discuss any needed changes and how to implement them, and 7) Share evaluation findings and success stories with partner organizations, community members and other project stakeholders.
