What You Will Do:
As a Care Manager II, you will collaborate with patients, families, physicians, and multidisciplinary teams to develop and execute individualized care plans from admission to discharge. Your expertise will drive patient flow, enhance safety, satisfaction, and facilitate smooth transitions across care settings. Responsibilities include comprehensive assessments, discharge planning, resource coordination, and acting as an advocate to ensure patients receive appropriate post-acute care. You will play a key role in removing barriers to timely discharge, supporting quality initiatives, and fostering a patient-centered approach.
Required Skills:
Active RN or LMSW licensure in Texas
Minimum of two years clinical experience, with at least one year in an acute care setting
Proficiency in case management, utilization review, MCG and InterQual criteria, and EPIC software
Strong analytical, critical thinking, and multitasking abilities
Excellent verbal and written communication skills
Ability to adapt in a fast-paced, high-stress environment and work independently
Knowledge of discharge planning, community resources, and managed care reimbursement
Demonstrated competence in clinical assessments and care coordination
Nice to Have Skills:
Certification in Case Management
Experience with hospital management systems and performance improvement initiatives
BLS certification
Background in acute hospital management or complex care settings
Preferred Education and Experience:
Bachelors degree in Nursing (BSN) or Masters in Social Work (MSW) preferred
Five or more years of experience, including prior success in a Care Manager I position or equivalent healthcare roles
Other Requirements:
This role requires travel within healthcare facilities and flexibility to arrive early (as early as 7:30 AM) and stay until all cases are completed, which may extend beyond standard hours
Start date: March 16, 2026
Hourly pay rate: $46.90
Willingness to participate in special projects and serve as a preceptor or orienter as needed
