Overview:
We are seeking a compassionate and experienced Registered Nurse - Integrated Care Manager (ICM) for a temporary assignment (approximately 3 months, with potential for extension). This position requires the ability to conduct face-to-face clinical assessments with members residing in Lynchburg area of VA and the candidates must reside closer to this area.
The RN ICM works collaboratively with members, families, providers, and the care team to optimize health outcomes, coordinate services, and support members across the care continuum. The RN will perform telephonic and in-person assessments, identify high-risk conditions, coordinate care plans, and assist with authorizations, referrals, and resource connections while ensuring compliance with regulatory and organizational standards.
