Pediatric Home Health Nurse (RN or LPN) – Make a Difference Every Day!

Pay:
$26–$35 per hour

  • LoTech (G-tube): $26–$31
  • HiTech (Trach/Vent): $30–$35

Family First Homecare (FFHC) is looking for compassionate and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to deliver high-quality, in-home nursing care to amazing pediatric patients!

KEY RESPONSIBILITIESĀ 

  • Accountable for providing care that is based on licensure/certification within the scope of practiceĀ Ā 

  • Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentationĀ Ā 

  • Evaluates the effectiveness of nursing service to the patient and family on an ongoing basisĀ Ā 

  • Consults with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisorĀ Ā 

  • Supports FFHC’s mission by mentoring and/or coaching new employees during patient specific orientationsĀ Ā 

  • Educates patients and their caregivers on topics related to patients' care needsĀ Ā 

  • Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requestedĀ Ā 

  • Continually strives to improve his/her nursing care skillsĀ Ā 

  • Attends and/or completes all mandatory training/ in-services as assignedĀ Ā 

  • Submits required personnel file documentation in a timely mannerĀ Ā 

  • Adheres to FFHC compliance and ethics expectationsĀ Ā 

  • Maintains patient and family confidentialityĀ Ā 

  • Maintains professional, supportive, and responsive interpersonal communication skillsĀ

  • LPN/LVN Nursing Degree.Ā Ā  Unencumbered LPN/LVN License in the state of employment or Multi-Statelicense through the Enhanced Nurse Licensure Compact where recognized.Ā 

  • 1 years of demonstrated experienceĀ 

  • Valid driver license with proof of insurance in applicable statesĀ 

  • Clear background screeningĀ 

  • Working knowledge of Nurse Practice ActsĀ 

  • Valid and current CPR card/certificate with in person instructor led hands on skills checkĀ 

  • Clear Hep B ScreeningĀ  in applicable statesĀ

KEY DETAILS:

** This role requires travel to patients' homes to deliver care. The work schedule may include varying shift lengths (8, 10, or 12 hours) and may require availability on any day of the week, Sunday through Saturday.
