What You'll Do
Review and assign patient referrals to the appropriate clinical team members.
Provide clinical leadership, mentoring, and support to ensure exceptional patient care.
Review patient diagnoses, medications, treatment plans, and overall clinical progress.
Assist clinicians with care planning, goal setting, and clinical decision-making.
Conduct quarterly clinical record reviews and provide recommendations for quality improvement.
Support compliance with regulatory standards and company policies.
Assist with developing, implementing, and evaluating clinical policies, procedures, and strategic initiatives.
Partner with leadership to drive quality outcomes and operational excellence.
Qualifications
Current professional clinical license in the state of Tennessee.
Graduate of an accredited healthcare program (BSN, DPT, MOT, or MSLP preferred).
Minimum of two years of healthcare leadership or supervisory experience (Home Health preferred).
Current CPR certification.
Valid driver's license and proof of automobile insurance.
