The Care Coordinator is the key point of contact in managing services for the child. Moreover, the Care Coordinator oversees and coordinates access to a variety of services an individual needs to help prevent hospitalizations, stay healthy, and maintainstability. The Care Coordinator provides comprehensive Care Management and coordination of services to eligible Medicaid/Medicaid Managed Care children (ages 0-21) to develop and maintain a plan of access and use of services and supports necessary for their health. The Care Coordinator helps children and caregivers get the care they need to improve the child's health.

Key qualifications include:
Bachelor's Degree in Human Services and/or related field.
Bilingual Spanish speaker/read/write
Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services.
Excellent writing, communication, and organizational skills.
A passion for serving children and families with special needs.
Ability to access and utilize ADP app on smartphone or other device for access to agency programs.
Physical/Travel Requirements of Care Coordinator Bilingual Spanish
Valid NYS Driver's License
Ability to travel to community-based agency offices, participant's communities, and homes within assigned catchment area
The Care Coordinator must be able to drive to external locations and work flexible hours as required to meet program needs with some evenings and/or weekends as identified.
