Youth Case Manager

Vital Health is a growing behavioral health and primary care center serving adults and children in Northwest Ohio. We provide trauma-informed, whole-person care and are looking for a Children's Case Manager to support youth in our day treatment program.

Schedule: School-day hours or 10am–8pm, Mon–Thurs.

What You'll Do:

• Provide case management for school-age youth
• Work with children in the clinic, community, and group settings
• Support behavior modification, life-skills, goal setting, and treatment planning
• Collaborate with families, schools, and community partners
• Complete timely documentation and maintain compliance
• Assist with transportation when needed

Requirements:

• 2+ years case management experience
• 2+ years behavioral health experience (youth preferred)
• QMHS experience preferred
• Active CDCA or CHW a plus
• Previous Ohio Medicaid provider number preferred
• Strong engagement, communication, organization, and computer skills
• Ability to work with culturally diverse youth and families
