Job description:

Job Summary: The Wraparound Facilitator works to meet the complex mental health challenges of children/youth and their families.

DUTIES AND RESPONSIBILITIES:

Engage the family

Assemble the child/youth and family team

Facilitate a child/youth and family team meeting every thirty (30) days, and as needed.

Facilitate the creation of a Wraparound Plan of Care, indicating a plan for anticipating, preventing and managing crisis, within the child/youth and family team meeting.

Work with the team to identify agency providers of services and other community resources to meet family and child/youth needs.

Make necessary referrals for children/youth.

Document and maintain all information regarding the plan of care, including revisions and team meetings.

Present the plan of care for approval by the family and team.

Provide copies of the plan of care to the entire team.

Monitor the implementation of the plan of care and revise if necessary to achieve outcomes.

Maintain communication between all child/youth and family team members.

Monitor the progress toward needs met and whether or not the referral behaviors are decreasing.

Lead the team to discuss and ensure the supports and services the child/youth and family are receiving continue to meet the caregiver and child/youth needs.

Educate new team members about the wraparound process.

Maintain team cohesiveness.

Participate in monthly treatment team meetings.

Gather & track program data and implementing policies & procedures.

Track compliance with all applicable regulations and preparing for survey & license reviews or audits.

Collaborate with comprehensive teams and community organizations to coordinate client treatment and make appropriate referrals.

Maintain and ensure client confidentiality at all times.

Assume other responsibilities as assigned

Background, Licensing Requirements

  • Bachelor’s degree in mental health, IDD or human resources/behavioral health related field AND DMH Community Support Specialist credential.
  • Training by the MS Wraparound Institute is highly preferred
  • Driver’s license and vehicle liability insurance

Skills:

  • Good overall clinical skills, ability to work productively and effectively.
  • Ability to function well as a member of a multidisciplinary team.
