The White Oak Post Acute is seeking a dedicated Wound Care/Treatment LPN!
Registered Nurse (LPN) - Treatment Focus
Responsibilities:
- Patient Assessment & Care: Perform comprehensive assessments, monitor vital signs, and document resident conditions, reporting changes to physicians and unit managers.
- Wound Care Specialist: Manage all aspects of wound care, including assessing skin breakdown, applying treatments, educating staff/residents, documenting wound status, and completing quality assurance reports.
- Medication & Treatment Administration: Administer medications and treatments as ordered, transcribing physician orders accurately.
- Care Plan Management: Implement and coordinate care plans, ensuring adherence to physician orders, facility policies, and federal/state guidelines.
- Supervision & Delegation: Oversee and guide nursing assistants (CNAs) in providing care, ensuring proper skin prevention programs and daily tasks are completed.
- Documentation: Maintain accurate, detailed, and timely clinical records and reports.
- Communication: Act as a liaison between residents, families, physicians, and the interdisciplinary team.
- Emergency Response: Provide prompt response to urgent physical and emotional needs and manage emergency situations.
- Education: Educate residents and families on self-care techniques and treatment plans.
Qualifications:
- Current, valid LPN license.
- Experience in long-term care or SNF setting preferred.
- Strong assessment, critical thinking, and organizational skills.
- Ability to work independently and as part of a team.
