Speech-Language Pathologist (SLP) – Skilled Nursing Facility (SNF) 

We are seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join a collaborative therapy team within a skilled nursing facility. This role focuses on providing high-quality rehabilitation services to short-term rehab patients and long-term residents, helping them improve communication, cognition, and swallowing function. 

The SLP will evaluate, treat, and monitor patients with speech, language, cognitive-communication, and dysphagia needs, implementing individualized plans of care under evidence-based guidelines. Strong clinical judgment, communication skills, and teamwork are essential to ensure positive patient outcomes. 

Key Responsibilities: 

  • Perform comprehensive evaluations for speech, language, cognitive-communication, and swallowing deficits 

  • Develop and implement individualized treatment plans under the supervision of facility guidelines 

  • Provide skilled therapy for residents with neurological, cognitive, or swallowing impairments 

  • Educate patients, families, and caregivers on strategies to improve communication and safe swallowing 

  • Monitor patient progress and communicate updates to the interdisciplinary team 

  • Accurately document evaluations, treatment sessions, and progress notes in EMR 

  • Collaborate with PT, OT, nursing, and case management to ensure coordinated, patient-centered care 

  • Maintain productivity while delivering high-quality rehabilitation services 

Ideal Candidate: 

  • Active SLP license in practicing state 

  • SNF or geriatric rehabilitation experience preferred 

  • Strong clinical reasoning and documentation skills 

  • Comfortable managing both short-term rehab and long-term resident caseloads
