Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); every 3rd weekend required; minor holidays as needed (flexible schedule)

Hours per Week: 40

Roles & Responsibilities
  • Provide skilled physical therapy services to home health patients, focusing on mobility, functional independence, and safety
  • Complete OASIS documentation accurately and promptly to meet regulatory requirements
  • Use Cerner EMR for charting, care coordination, and communication with the interdisciplinary team
  • Assess patient status, design and implement treatment plans, and progress interventions based on functional goals
  • Collaborate with PT, OT, and Speech colleagues — and supervise rehab aides/OTAs when appropriate
  • Cover an assigned geographical region, managing travel between patient homes efficiently
  • Participate in orientation and continuing learning to ensure consistent, high-quality patient care
Qualifications
  • Current PTA license/certification as required by state regulations
  • BLS certification (required)
  • Minimum 2 years of clinical experience (job listing indicates 2 years); at least 1 year experience working with PT/OT/SLP populations
  • Experience with OASIS documentation (required)
  • Familiarity with Cerner or similar EMR systems for clinical charting
  • Home health experience preferred but not required
  • Ability to work professionally in business attire with white lab coat and adhere to uniform policy
  • Willingness to work every 3rd weekend and cover minor holidays as needed
Knowledge & Skills
  • Proficient in OASIS documentation and compliance requirements
  • Charting and care coordination using Cerner EMR
  • Clinical assessment and treatment planning for ortho, neurological (stroke), spinal and joint rehab
  • Patient safety, mobility training, fall risk assessment, and home environment adaptation
  • Strong communication and teamwork across PT, OT, and Speech disciplines
  • Time management and regional scheduling for efficient patient visits
  • Supervision and delegation skills when working with aides or OTAs
