Job Title: Physical Therapy Assistant Profession: PTA (Physical Therapy Assistant) Specialty: Home Health, Physical Therapy Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum of 2 years in a healthcare setting required. Home health experience preferred. License: Current Physical Therapy license required Certifications: BLS Certification required Must-Have: OASIS experience required. EMR HomeCare Home Base (HCHB) experience preferred. Description: Looking for a candidate for leave of absence coverage. Profiles will go directly to the unit manager for consideration. Candidates may not have been directly employed by any facility affiliated with the organization within the past year. Candidates who were employed PRN through outside firms will be considered. Good oral and written communication skills are essential. Strong organizational skills are required. Mileage reimbursement is available per guidelines. Candidates will provide physical therapy services to patients according to a written physician's plan of care. This includes assessing, evaluating, and updating therapeutic and rehabilitative status. Participation in developing the total plan of care is required. Evaluating the home environment and assessing for muscle strength, mobility, gait, and range of motion is necessary. Directing physical therapy treatment will be part of the role. Instructing patients and caregivers in the use and care of therapeutic appliances is expected. Determining priority needs for physical therapy is also a part of the responsibilities. Reporting the patient's reaction to treatment or changes in condition to the physician is required. The candidate will initiate physical therapy programs and instruct other personnel and family members. Contributing to case conferences is important. Training patients in the use of prosthetic devices may be required. Supervision of homemakers or aides involved in the patient's rehabilitation is necessary. Identifying patient and family needs for other home health services and making referrals as needed is part of the role. Preparing and submitting clinical and progress summaries based on goal attainment is required. Observing, recording, and reporting information on the patient's condition is essential. Providing physical therapy consultation to caregivers when indicated is expected. Delivering in-service education programs for nursing personnel as needed is part of the responsibilities. Participation in peer consultation processes is expected. Ongoing staff competency assessment will be part of the role. Compliance with all relevant regulations and licensure requirements is necessary. Performing other duties as assigned and requested will be a part of the position. Cancellation policy allows for the cancellation of shifts as needed.
