Responsibilities

• Implement PT treatment plans under the supervision and direction of the Physical Therapist.
• Deliver skilled PT interventions targeting mobility, strength, balance, and functional independence.
• Document patient progress and treatment notes accurately and within required timeframes.
• Collaborate with the interdisciplinary team including OT, SLP, and nursing staff.
• Educate patients and families on home exercise programmes and safe mobility strategies.
• Meet facility productivity standards while maintaining high-quality care.

Requirements

• Current, unencumbered PTA licence/certification in New Hampshire required.
• Graduate of an accredited Physical Therapist Assistant programme.
• Minimum 1 year of SNF or post-acute rehab experience preferred.
• Current BLS/CPR certification.
• Strong documentation skills and ability to meet productivity targets.
