Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF) 

We are seeking a dedicated and skilled Physical Therapist Assistant (PTA) to join a collaborative therapy team within a skilled nursing facility. This role focuses on providing high-quality rehabilitation services to short-term rehab patients and long-term residents, helping them regain mobility, independence, and confidence in daily activities. 

The PTA will implement individualized treatment plans developed by the supervising Physical Therapist, delivering skilled interventions that improve strength, balance, gait, and functional mobility. This position requires strong clinical judgment, communication skills, and the ability to manage a productive caseload in a supportive, team-based environment. 

Key Responsibilities: 

  • Provide skilled physical therapy interventions under the supervision of a licensed PT 

  • Assist patients with transfers, gait training, balance, strength, and endurance exercises 

  • Implement fall prevention strategies and assist with adaptive equipment recommendations 

  • Educate patients and caregivers on safe mobility and home exercise programs 

  • Monitor patient progress and communicate updates to supervising PT 

  • Accurately document treatment sessions, progress notes, and discharge summaries in EMR 

  • Collaborate with interdisciplinary team members including OT, SLP, nursing, and case management 

  • Maintain productivity while ensuring patient-centered care 

Ideal Candidate: 

  • Active PTA license in practicing state 

  • SNF or geriatric rehabilitation experience preferred 

  • Strong clinical reasoning and communication skills 

  • Comfortable managing both short-term rehab and long-term resident caseloads
