Minimum Qualifications
- Associate degree in Physical Therapy from an accredited educational program.
- Active Illinois PTA license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience working in a skilled nursing facility or long-term care setting preferred.
- Experience treating post-acute, geriatric, or medically complex patients preferred.
- Ability to work independently under PT direction and document per facility and regulatory requirements.
- Client-specific requirement: ability to work 5x8 schedule, supporting days 0830–1630.

- Shift hours: 0830–1630.
- Schedule: Monday through Friday, five 8-hour shifts per week.
