Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans that target mobility, strength, balance, and function
- Deliver skilled therapy services to help patients regain independence and meet rehab goals
- Monitor and document patient progress accurately and in a timely manner to support care continuity
- Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes
- Educate patients and caregivers on therapy goals and effective home exercise programs to support long-term recovery
Qualifications
- Active California Physical Therapist license (required)
- Experience in a Skilled Nursing or rehabilitation setting preferred
- Strong clinical judgment and excellent communication skills
- Ability to work independently while also contributing effectively as part of a team
Knowledge & Skills
- Physical Therapy principles and patient-centered care
- Patient evaluation and individualized treatment planning
- Skilled therapeutic interventions to improve mobility, strength, balance, and function
- Accurate clinical documentation and progress monitoring
- Interdisciplinary collaboration and patient/caregiver education
