Roles & Responsibilities
  • Implement physical therapy treatment plans developed by the supervising Physical Therapist to help patients regain mobility and function
  • Provide skilled therapeutic interventions focusing on mobility, gait training, strength, balance, and functional activities
  • Assist patients with therapeutic exercises and functional mobility tasks to promote independence
  • Monitor patient response to treatment and communicate progress or concerns to the supervising PT
  • Complete accurate and timely documentation of treatment sessions and patient progress
  • Collaborate with occupational therapists, speech therapists, nursing staff, and other care team members to deliver coordinated care
  • Educate patients and caregivers on exercises, safety techniques, and mobility strategies to support home readiness
  • Follow facility policies, state regulations, and professional standards to maintain a safe, compliant practice environment


Qualifications
  • Current Physical Therapist Assistant (PTA) license in the state of California
  • Graduate of an accredited Physical Therapist Assistant program
  • Experience in a Skilled Nursing Facility or post-acute care setting preferred
  • Strong patient care and clinical skills with a focus on geriatric populations
  • Effective communication and time-management abilities
  • Ability to work independently while collaborating within a multidisciplinary team


Education
  • Graduate of an accredited Physical Therapist Assistant program


Knowledge & Skills
  • Solid knowledge of mobility, gait training, strength and balance interventions
  • Skilled in safe patient handling and transfer techniques
  • Proficient in clinical documentation and progress note accuracy
  • Ability to assess patient response and adjust care communications to the supervising PT
  • Experience collaborating with interdisciplinary teams and coordinating care
  • Patient education skills to teach exercises, safety practices, and functional strategies
