Job Title: Physical Therapist Profession: Physical Therapist Specialty: Rehab Therapy Duration: Full-Time Shift: Days Hours per Shift: 40 hours per week Experience: Work experience not required other than clinical experience within the educational training program. License: Must hold or be eligible to receive a Physical Therapist license in the State where services will be provided. Certifications: Must be certified in CPR or must become certified within the first 90 days of employment. Must-Have: - Must be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows: Minimum of a Bachelor's degree, if graduated prior to 01/01/2016; Doctorate of Physical Therapy required, if graduated after 01/01/2016. - International candidates must be a graduate of an accredited school and possess a minimum of a Bachelor's degree in Physical Therapy. - Doctorate of Physical Therapy degree preferred. - Must maintain all certifications and licenses according to the expiration date. - American Physical Therapy Association membership preferred. - If assigned to drive a company vehicle, a valid driver's license is required. - If assigned to drive a personal vehicle, proof of insurance is required in addition to a valid driver's license. - Must be willing to work at other locations as assigned. Description: Provides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment. Evaluates and treats patients in accordance with the order from the physician and provides treatment through a variety of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of the patient's life. Develops and implements an individualized treatment plan based on patient needs and containing achievable functional goals. Revises treatment plans as needed throughout the treatment process. Instructs and collaborates with Physical Therapist Assistants regarding treatment procedures to be utilized and supervises patients being treated by the Assistant. Establishes and educates patients, families, and other caregivers in exercises and therapeutic home programs to be continued following discharge. Completes appropriate documentation in a timely manner. All care is provided in accordance with the philosophy and essential functions of the organization, the rehabilitation department, and the applicable practice acts and standards.
