Roles & Responsibilities
- Assess residents through thorough evaluations to identify physical therapy needs
- Develop and implement individualized treatment plans that improve mobility and independence
- Provide skilled therapy to enhance strength, balance, and functional outcomes
- Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and on time
- Collaborate with interdisciplinary team members to coordinate patient care
- Educate patients and caregivers on therapeutic exercises and safety techniques
- Ensure compliance with Medicare, Medi-Cal, and facility regulatory standards
Qualifications
- Active California Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Previous skilled nursing facility or post-acute experience preferred but not required
- Strong documentation and time management skills
- Ability to work independently in a dynamic environment and collaborate with a team
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Patient assessment and evaluation for geriatric and post-acute populations
- Treatment planning and skilled therapeutic interventions
- Skilled documentation practices and regulatory compliance (Medicare, Medi-Cal)
- Interdisciplinary collaboration with OT, SLP, nursing, and physicians
