Shift Type: Monday-Friday, 7:00 AM - 3:00 PM (Day shift)

Hours per Week: 40

Roles & Responsibilities
  • Assess and evaluate patients with physical functioning disorders and wound care to create meaningful, evidence-based plans of care
  • Deliver therapeutic interventions including manual techniques, therapeutic exercise, ambulation training, modalities, and assistive device training
  • Provide skilled wound care and adjust treatments based on patient progress and standardized assessments
  • Document accurately and thoroughly in the electronic medical record, including treatment minutes and discharge summaries
  • Collaborate and communicate with the interdisciplinary team — you’ll be the go-to for patient progress and care coordination
  • Supervise and support PTAs, rehab aides, and students to build a strong therapy team
  • Procure necessary DME and supplies per regulatory and clinical practice requirements
  • Participate in facility and company trainings, patient care conferences, utilization review, and rehab meetings to maintain top-quality care
Qualifications
  • Active Missouri state Physical Therapy license (required) — compact allowed
  • Minimum 1 year of PT experience (LTC or rehab experience required); new grads welcome with Level II clinical experience (not just Level I observation)
  • Experience with electronic documentation and accurate recording of treatment minutes
  • Knowledge of regulatory and clinical practice standards and adherence to APTA/state practice acts
  • Comfort supervising PTAs/rehab aides/students and working within productivity expectations
  • Strong communication skills for patient/family education and interdisciplinary collaboration
Education
  • Graduation from an accredited Physical Therapy program (DPT preferred) and eligibility for Missouri licensure
Knowledge & Skills
  • Patient assessment and development of effective plans of care
  • Wound care expertise and outcome evaluation
  • Therapeutic interventions: manual techniques, therapeutic exercise, modalities, ambulation training
  • Use of assistive/supportive devices and DME procurement
  • Accurate electronic documentation and regulatory-compliant record keeping
  • Discharge planning and ability to produce thorough discharge summaries
  • Familiarity with Medicare A/B, managed care, productivity targets, and scheduling
  • Group therapy delivery and supervision skills
